Application form and bank deposit slip by clicking on below link.
2.
Deposit amounting to Rs. 125.00 (one hundred twenty five rupees) in any Brach
of HBL or Silk Bank.
3.
Forward your application form along with original bank deposit slip (ATS Copy),
photocopy of national identity card and 2 x passport size photographs (white
background) direct to ATS Headquarters. Address is also mentioned on
application form.
